Developing “Aged Care Know How” supports people to think about, talk, and plan for their own well-being. Life often presents us with setbacks. Being prepared with the right information, and the right questions, keeps you in control.

The Living Well at Home session starts with a short presentation, and is followed by an interactive fun workshop where people make a “wellness map.” People often need information and support to get the right help at home – and having a map of your needs and preferences is a great starting point.

We work together to simplify and disentangle the many messages and programs available for older people who need support at home.   We share handy hints in a safe environment.  Working with My Aged Care to get your needs (or someone else’s needs) met will become a whole lot easier.

We share top tips about home based, subsidised supports available via the Commonwealth Government’s My Aged Care and the State Government hospital system.

Did you know you can  get help at home to “restore or regain” your well-being after a hospital stay?  If you have a set back at home, another form of support is available (through My Aged Care).  Waiting times for a home care package are an issue for many.  People can (while recovering from a setback at home), get back to wellness faster by asking for a short term restorative care package.

At the end of the session, we offer support people who want to discuss their personal situation.

We also work with service providers and volunteer based organisations sharing key messages and approaches supporting people to age independently at home, connected to their community.
