Are you between the ages of 18 and 65 and need practical help and support with your usual activities of daily living because of your health condition?

For example, getting groceries, cooking at home, getting to important appointments and getting outside for some fresh air and exercise.

If this is you or if you are caring for someone who has a health condition, we would like to hear from you about:

– Whether or not you have been able to get the help you needed?

– How did you find out about the services? Was it easy to find the information and to contact service providers?

Your feedback will help improve the Tasmanian Home and Community Care Program (HACC).

This program is to support people aged between 18 to 65, living at home, who are ill or recovering from an illness or injury and need support performing their usual activities of daily living.

This includes people returning home from hospital when they need some practical support to be at home as they recover. Including people with mental illness, and also people receiving palliative care to remain at home for as long as possible.

This consultation session is jointly hosted by Health Consumers Tasmania and the Tasmanian Department of Health.

Morning/afternoon tea is provided. We appreciate your advice on dietary requirements.

A total sitting fee is available of $60 per person for attendance at a consultation session.
